Reservation Form

1) Fill the reservations form below.
2) Read and acknowledge the rental contract.

I understand that a minimum deposit of $150.00 (or the total amount if less) will be charged to my credit card, and that deposits are nonrefundable. The remainder of the balance will be charged upon completion of the service. Reservations made by check will be confirmed only after payment is received.

The order will be revised by office personnel. In the event that incorrect amounts are entered, the correct sum will be processed and the customer notified.

American Limousine Service is not responsible for incorrect information entered in this form, which may cause delays or inability to provide the service. In that event, any fees charged will not be refunded.
